Applications Must Include
- Application Face Page:
- The Application Face Page is provided in Microsoft Word format.
- Complete the Application Face Page.
- When finished, save your work in a new file. To do this:
- Click File -> Save As
- Enter your last name and first initial
(e.g.: Joe Smith would be smithjFP.doc)
- Click OK
- Print a copy of the Face Page and complete all required signatures.
- Applicant’s curriculum vitae and bibliography
- NIH Format
- Two page limit
- Save your file in Microsoft Word format
- Click File -> Save As
- Enter your last name and first initial plus "CVBIB"
(e.g. Joe Smith would be smithjCVBIB.doc)
- Training plan (residents and fellows only)
- One page description of training to occur during fellowship.
- Description of the research proposal.
- Limited to six (6) single-spaced pages.
- Arial Font, minimum size 10 point
- Margins: Left 1.5 inches, all other 0.75 minimum
- The proposal should include the following:
- Title of the research project
- Abstract (200 word limit)
- Significance of the research
- Background information
- Preliminary data/ Progress Report if competitive renewal
- Study design
- Methods
- Data analysis
- References cited (2 page maximum, in addition to the 6 page limit).
- Save your file in Microsoft Word format
- Click File -> Save AS
- Enter your last name and first initial plus “PROPOSAL”
(e.g. Joe Smith would be smithjPROPOSAL.DOC)
- Institutional approvals (e.g., IRB, Animal Studies Committee) are not required for the initial submission of the SIS Fellowship applications, but if the grant is awarded, documentation of approval must be provided.
- Documentation of completion of an approved research protections course for all participants if the proposal includes human studies, must be completed at the time of funding.
- Letter of support from sponsor (if indicated).
- Letter of support from the department chair.
- Letters of endorsement from one additional member of the Surgical Infection Society.
Applications that do not include all the required elements will not be considered.
Renewal competitive application for a one-year extension of the Fellowship will be considered. A section highlighting work accomplished and in progress should be included in proposal (must be included in the 6-page limit of the research proposal). All recipients for research support from the SIS are encouraged to submit their work for consideration of presentation at the SIS Annual meeting. All publications based on work supported by SIS fellowship grants should acknowledge that support.
Submitting the Application
Application (Items #1-9 above) must be received by January 11, 2013.
